【 要旨 】 超音波内視鏡（EUS）は，胆膵疾患の画像診断に不可欠なモダリティーとなっており，専用スコープ，超音波観測装置とも十分な画質と多彩な機能を装備したものが市販されている．EUSガイド下に処置具を用いて行う診断，治療はinterventional EUSと呼ばれる．このうち，EUS‒FNA（超音波内視鏡ガイド下穿刺吸引術）は，これまでアプローチ困難であった部位に対する生検診断としての有用性がほぼ確立され，各社から穿刺針が市販されている．EUSガイド下治療としては，膵周囲液体貯留ドレナージ，EUSガイド下ドレナージ術などが行われている．しかし処置具については，現状ではEUS‒FNA用穿刺針やERCP関連手技用のものが代用されており，安全かつ確実な手技となるためには専用処置具の開発が急務である．

[ Summary ] Currently endoscopic ultrasonography (EUS) plays an important role in diagnosis and treatment in the pancreatobiliary field. EUS‒guided fine needle aspiration (FNA) has been established as a safe procedure. EUS is used in the management of pancreatic fluid collection, biliary and pancreatic duct drainage, and other conditions. Further device development would enable the expansion of its role and prompt its widespread use in clinical practice.
