【 要旨 】 超高齢化社会に伴い，高齢者の大腸癌に対する内視鏡治療の需要は今後も高まっていくと予想される．安全で確実に内視鏡治療を行うためには，術中のみならず術前・術後の患者管理が重要である．当院では，2007年よりクリニカルパスを導入し活用することで，万が一偶発症が起きた場合にも迅速に対応できる標準的な周術期管理を確立してきた．しかしながら，高齢者では若年者と比較してさまざまな基礎疾患を有する割合が高く，よりいっそう慎重な周術期管理が必要となる．当院の高齢者に対する内視鏡治療の周術期管理における注意点や工夫について解説した．

[ Summary ] By Japan having broken into the super aging society, it is thought that the demand for endoscopic treatment in the elderly colorectal cancer patients continues increasing more and more. Endoscopic treatment involves not only intraoperative management but also postoperative safety. In our hospital, a clinical pathway has been applied since 2007 to all patients who undergo colorectal endoscopic treatment. Utilization of such a clinical path facilitates quick and proper management of complications including postoperative perforation and bleeding. However, very careful perioperative management is essential in elderly patients, because they have a lot of underlying disease.
