【 要旨 】 sedationは内視鏡治療（とくにESD）において必須である．適切にsedationを行うことで患者の苦痛軽減が得られるだけではなく，術中の安定的な視野確保，治療の安全性向上の面からもきわめて重要である．その一方でsedationに伴う偶発症も知られており，その対策も十分になされなければならない．十分な薬剤特性の熟知，適切な術中のモニタリングだけではなく，実際に偶発症が起きた際にすぐに対応できるような環境整備，急変した際のシミュレーション訓練など，常日頃の準備が大切である．

[ Summary ] Sedation is an essential tool in therapeutic endoscopy, especially in endoscopic submucosal dissection. The purpose of sedation is not only to relieve the patient of discomfort, but also to improve the conditions during an endoscopic procedure. However, some complications related to sedation are known. Knowledge about the pharmacological profiles of sedative agents and appropriate monitoring are necessary to maximize the likelihood that the desired level of sedation is targeted accurately. The preparation of instruments or medication for resuscitation is also necessary.
