【 要旨 】 | 2016年厚生労働省の「がん予防重点健康教育およびがん検診実施の指針」により対策型胃がん検診にて内視鏡検査が可能となった.胃内視鏡検診はおもに経口内視鏡で行われるが,近年,経鼻内視鏡も導入されその受容性から検査件数が増加し,経鼻内視鏡は胃がん検診受診率向上にかなり寄与している.一方,苦痛の少ない内視鏡に対する受診者ニーズと受容性や満足度の点で鎮静内視鏡の希望者も増加しているが,安全性という観点から胃内視鏡検診に導入することは難しい.経鼻内視鏡は,安全性,受容性,検査精度などの点で経口内視鏡と差がなく,今後,胃内視鏡検診において経鼻内視鏡はますます重要な位置を占めると考えられる. |

[ Summary ] | According to guidelines established by the Ministry of Health, Labor and Welfare in 2016, endoscopy can be used for population‒based screening for stomach cancer. Although endoscopic screening is mainly performed using oral endoscopy, recently, transnasal endoscopy has also been introduced, and the number of examinees is increasing due to its acceptability. Transnasal endoscopy contributes considerably to improving the percentage of examinees screened for stomach cancer. On the other hand, demand for sedative endoscopy is also increasing in view of examinees' needs for endoscopy with less distress and greater acceptability and satisfaction. However, from the viewpoint of safety, it is difficult to use it for endoscopic screening. Transnasal endoscopy does not differ from oral endoscopy in terms of safety, acceptability, and diagnostic accuracy. Henceforth, transnasal endoscopy is expected to occupy increasingly important roles in screening for stomach cancer. |
