【 要旨 】 対策型胃がん検診では，これまでの胃X線検診に加えて内視鏡検診に有効性が認められたことから，内視鏡検診への期待が膨らんでいる．内視鏡検診では，医師の労働資源や費用を有効に活用して安全で精度の高い検査として導入するための体制づくりが必要不可欠である．当センターでは，視野角140°である内視鏡検査の弱点を分析して，精度が高く効率的な観察・撮影方法を作成した．その観察・撮影方法を用いて発見された経年発見胃癌321例について，検査間隔と深達度について検討したところ，逐年検診では早期癌率：97.4％（粘膜内癌率：81.1％），隔年検診では早期癌率：94.8％（粘膜内癌率は65.5％）であり，逐年と隔年検診で早期癌率に有意差はないが，粘膜内癌率が逐年で高率であった．

[ Summary ] In mass screening for gastric cancer, endoscopic examination may be introduced and effectively utilized ; however, it requires a large work force of doctors and is expensive. In addition, a system is required to ensure high precision. We detected 321 cases of gastric cancer in our center ; the incidence of early stomach cancer did not differ between the annual and biennial examinations ; however, the endoscope examination revealed a high incidence of possible intramucosal carcinoma in the annual examination.
