【 要旨 】 感染管理は内視鏡におけるもっとも基本的かつ重要な課題の一つである.大腸内視鏡の感染管理は,内視鏡機器の再生処理を含めて,上部消化管内視鏡と変わりはない.しかし,大腸内視鏡には (1)便汁によるベッドや床などの汚染,(2)感染性腸炎の存在,(3)芽胞形成菌(Clostridium difficileなど)の問題などが存在し,感染対策は上部消化管内視鏡以上に慎重に行う必要がある.現在,わが国では多くの施設においてガイドラインを遵守した適切な内視鏡機器の再生処理が行われるようになった.一方,内視鏡における清潔操作や内視鏡従事者に対する感染予防については,十分な対策が行われていない.とくに,内視鏡医の関心が低いことが大きな問題である.

[ Summary ] Infection control is an essential problem in endoscopy. Basically, the procedure of infection control does not differ between colonoscopy and upper gastrointestinal endoscopy. However, infection control in colonoscopy requires careful attention because of the following : 1. During colonoscopy, patients' feces contaminate the beds and floors, 2. We sometimes encounter patients with infectious colitis who undergo colonoscopy without information of the fear, 3. Spore forming bacteria such as Clostridium difficile sometimes exist in the colon. Presently, appropriate reprocessing of endoscopes and endoscopic equipment is carried out in most of the Japanese medical institutions ; however, consciousness of cleanliness is not so high among the engaging staff in endoscopy, especially among endoscopists. Therefore, some inadequately behaved staff, which unconsciously contaminates surroundings in an endoscopic room, is sometimes recognized. Furthermore, it is important to wear personal protection items such as a pair of rubber gloves, a mask, a pair of glasses (or a pair of goggles), and a gown for prevention of "patient?to?staff" infection. Endoscopists should not leave a measure of infection control during endoscopy to co?medical staff only, but should divide its responsibilities and take on it themselves.
