【 要旨 】 ダブルバルーン内視鏡はプッシュ式小腸内視鏡のもつ問題点に着目し,その解決方法を考えていくうちに発想した.比較的単純な原理だが,内視鏡挿入における小腸がもつ解剖学的構造のデメリットをメリットに変えることができたため,その挿入性は飛躍的に改善した.優れた挿入性に加え,優れた操作性も有していたため,精査・治療のできる小腸内視鏡として世界に普及した.小腸内視鏡として以外に術後腸管におけるERCP,挿入・治療困難例の大腸内視鏡としても有用である.小腸疾患を中心とした消化管疾患の診療に革命をもたらすことができた.
新たな開発の元はニーズから生まれる.問題の認識,分析,工夫,諦めない努力が成功につながる.

[ Summary ] Double‒balloon endoscopy was developed to solve problems associated with push enteroscopy. The principle of double‒balloon endoscopy is simple, but has dramatically increased the intubation depth into the small intestine. The principle has changed the anatomical disadvantage of the small intestine into an advantage for endoscope insertion, based on its freely mobile nature in the abdominal cavity. Since double‒balloon endoscopy has excellent maneuverability with interventional capability, in addition to being capable of deep intubation, it is widely used. It is also useful for endoscopic retrograde cholangiopancreatography (ERCP) in patients with surgically altered anatomy, and can be used with a colonoscope in cases of difficult intubation and/or treatment. It has revolutionized the management of several gastrointestinal diseases, mainly in the small intestine.
Innovative developments often arise from needs. Recognition and analysis of problems leads to new ideas. In order to succeed, do not give up.
