【 要旨 】 ERCPは，1969年本邦から世界中に広められた．その後，内視鏡的胆管結石除去術の基本となるESTも本邦において開発され，現在では多くの施設で第一選択の手技となっている．さらに，胆膵内視鏡分野では，内視鏡的胆管ドレナージ術，術後再建腸管に対するバルーン内視鏡による胆膵内視鏡処置，胆管・膵管内視鏡などさまざまな技術革新がなされてきた．今後さらなる技術革新が本邦から発信されることが期待される．

[ Summary ] Endoscopic retrograde cholangio‒pancreatography (ERCP) was introduced in 1969 in Japan, and its use has spread worldwide. Endoscopic sphincterotomy (EST) also began in Japan, and is now widely accepted as first‒line endoscopic treatment of bile duct stones. There have been many technological innovations in the field of pancreatobiliary endoscopy, such as endoscopic biliary drainage, balloon endoscopy‒assisted ERCP in patients with surgically altered anatomy, peroral pancreatoscopy and cholangioscopy, and others. It is expected that technological innovation will be ongoing in Japan.
