| 【 要旨 】 | 欧米では，消化器内視鏡を施行する際には，全例に鎮静薬あるいは鎮痛薬が投与されている．一方わが国では，時間がかかる内視鏡治療や胆膵内視鏡などでは従来より鎮静を行ってきたが，通常の上部・下部消化管内視鏡では鎮静なしで施行することが多かった．近年，通常内視鏡においても鎮静を希望する患者が増加しており，「がまんする内視鏡」から「安楽な内視鏡」への流れに逆らうことはできない．しかし，鎮静下内視鏡を行う場合には，(1) 使用する鎮静薬や起こりうる偶発症の知識，(2) モニタリング機器の購入，(3) 拮抗薬や救急カートの準備，(4) リカバリー室の設置，(5) 患者に対する十分なインフォームド・コンセント（IC），が必要である． | |

| [ Summary ] | There should be no hesitation to administer sedation during endoscopy for patients who request it and for those who are suspected to have a low fortitude for the procedure without sedation. For the successful performance of sedation, it is important to possess sufficient knowledge of, and have experience in, the administration of sedative drugs. It is also important to be aware of the potential for suppression of respiration and changes in circulatory movement during the procedure. Additionally, it is essential to monitor the patient's blood oxygen concentration, pulse rate, electrocardiogram, and blood pressure. Furthermore, an emergency cart should always be implemented in the endoscopic division in order to prepare the antagonists for sedative drugs etc., and a substantial recovery room should be available. For endoscopic reservations, careful informed written consent for the procedure with sedation is required from all patients. | |
