【 要旨 】 小児の内視鏡領域において,上部消化管異物は比較的よく遭遇する病態の一つである.身近にある飲み込みやすいものを誤飲する傾向にあり,ほとんどが6歳未満の未就学児で,3歳以下の乳幼児が半数以上を占める.消化管異物のほとんどは自然排泄するとされているが,消化管穿孔や閉塞などを起こす症例も報告されており,異物の種類や形態・存在部位・誤飲後の経過時間などから速やかに内視鏡的異物摘出術の緊急性を判断する必要がある.また,成人と比較し小児の消化管は内腔が狭く壁が薄く脆弱であるため,内視鏡的異物摘出時にはより慎重な操作が求められる.

[ Summary ] In the field of pediatric endoscopy, foreign bodies in the upper gastrointestinal tract are relatively common. Children tend to accidentally swallow easy‒to‒swallow items found in their environment. Almost all of these patients are preschool children less than 6 years of age, and more than half are children ages 3 years or younger. Nearly all foreign bodies in the gastrointestinal tract are excreted spontaneously ; however, cases of gastrointestinal perforation and obstruction have been reported. Physicians should immediately assess the urgency of endoscopic removal of foreign bodies based on their types, forms, locations, and the time elapsed since accidental ingestion. In addition, the gastrointestinal tracts of children have a narrow lumen with a thin and weak wall compared to adults, and a more careful procedure is therefore required for the endoscopic removal of foreign bodies in children.
