【 要旨 】 大腸カプセル内視鏡は2014年1月に保険収載された.カプセルを飲み込むだけで検査を完遂可能という点で,大腸癌検診受診率の向上に寄与することが期待される.現在,第二世代大腸カプセル内視鏡が使用されている.第二世代大腸カプセル内視鏡の6mm以上のポリープに対する感度は海外で84~91%と報告されている.その後,本邦で行った治験では前処置法の改良を行い,その感度は94%まで向上した.一方,同様のレジメンで行った当施設のみの検討では,直腸病変に限定すると感度は37.5%と低い結果であった.前処置の問題など解決すべき点はまだ多いが,今後大腸癌スクリーニングにおける重要な役割を担っていく有用なモダリティの一つである.

[ Summary ] Colon capsule endoscopy (CCE) for colorectal examination can be availed under public insurance, based on a decision made by the Japanese ministry in April 2014. It is expected that CCE will be widely used in colorectal screening due to its lower degree of invasiveness. Moreover, the rat of colorectal cancer screening will also increase. The second generation of CCE, CCE-2, was released in the year 2009 in European countries. Recent overseas reports have documented the sensitivity of CCE-2 in detecting polyps with a diameter of > 6 mm as 84-91 %. Moreover, a prospective multicenter study in Japan, including our facility, found CCE-2, with a sensitivity of 94 %, to be a useful method in colorectal screening for Japanese patients. In our prospective study involving 48 patients, CCE-2 was found to have a sensitivity of 85.4 % in detecting colonic polyps. This study also found a low ratio of detection of rectal polyps (37.5 %, 3/8) due to the blowing out of booster, especially at the lower rectum. Caution should therefore be exercised in interpreting the findings with CCE-2 for rectal lesions. In the future, more trials for CCE-2 that aim at decreasing the booster are necessary to make it a useful modality for noninvasive screening of colon and rectum.
