【 要旨 】 肝門部胆道狭窄に対するステンティング方法は未だコンセンサスが得られていない．QOLの点から経乳頭アプローチが第一選択となりつつあるが，ドレナージ領域(片葉か両葉か)，ステントの材質〔plastic stent（PS）かmetallic stent（MS）か〕については意見が分かれている．現状では狭窄の状態や医師の技量により最適なドレナージ方法が選択されているが，術者はそれぞれのドレナージ方法における利点や欠点について熟知する必要がある．MSによる両葉ドレナージ術の方法についてはpartial stent-in-stent（SIS）法とside-byside（SBS）法がありどちらも一長一短があるが，MSの改良に伴い手技が高率に完遂できるようになってきている．

[ Summary ] Biliary stenting for malignant hilar biliary obstructions is controversial. Endoscopic biliary drainage has become the first choice treatment for maintaining quality of life. However, there are many different opinions among clinical technicians concerning questions involved. These questions include how many stents should be placed (unilaterally or bilaterally), and which stents are most suitable (metallic stents or plastic stents). Bilateral biliary drainage of metallic stents for unresectable malignant hilar biliary obstruction can be performed with stent-in-stent or side-by-side techniques. Both the stent-in-stent and side-by-side techniques have their own merits and problems. Bilateral metallic stent placement has been achieved in most of cases with new improvements in metallic stents.
