| 【 要旨 】 | 慢性膵炎の無痛期では脂肪食制限は必要なく，有痛性の場合や十分な消化酵素薬補充でも反応しない脂肪便の症例に脂肪食を制限する．その場合，膵リパーゼの非存在下でも吸収可能な中鎖脂肪酸（MCT）は選択肢の一つであるが，副作用に留意しつつ使用する． わが国で使用される経口蛋白分解酵素阻害薬には十分なエビデンスはないが，疼痛軽減の可能性がある．高力価パンクレアチン腸溶性製剤が認可され，非代償期の慢性膵炎の治療の中心となる．また今後，成分栄養剤による慢性膵炎の管理が一つのオプションとなりうる． |

| [ Summary ] | It is no longer necessary to restrict fat ingestion for the painless chronic pancreatitis without steatorrhea. Fat restriction is necessary only for patients are fractory to enzyme replacement or for bouts of pain. Although medium chain triglyceride (MCT) produces adverse effects, e. g. meteorism, nausea, or vomiting, MCT is often chosen because nutrients are absorbable in the absence of pancreatic lipase. There is little evidence to indicate oral protease inhibitor may provide some degree of pain relief. Enteric coated pancreatin preparations are now available in Japan. All types of exocrine pancreatic insufficiency should be treated with this preparation. Elemental diets may be considered as a form of effective treatment for chronic pancreatitis in the near future. |
