【 要旨 】 十二指腸の腫瘍性病変はまれな疾患であり，通常の内視鏡検査で遭遇することは少ない．十二指腸腺腫・癌は隆起主体の肉眼型を呈することが多く，大腸病変と類似する傾向にあり，胃癌と異なり存在診断や範囲診断に苦労することはほとんどない．ただ，腺腫と癌を判別することは現時点では非常に困難であり，今後の研究に期待したい．そして，希少疾患ゆえに，さまざまな基準や標準的治療が定まっていないのが現状であり，今後の研究の進捗には「集約化」が重要な因子になると考えられる．もし，十二指腸腫瘍性病変を発見または疑ったなら，速やかに専門施設に紹介することが望ましい．

[ Summary ] Duodenal tumors are rare and there is usually little opportunity to observe them in daily medical practice. Duodenal adenomas and carcinomas frequently present an elevated morphology through endoscopic findings and can be detected easily with clear margins. However, it is very difficult to determine whether they are adenomas or carcinomas even if chromoendoscopy and NBI magnification are applied to the diagnostic process. There are no established criteria for standard treatment or curative resection at the present time. Centralization is considered to be important for the development of further investigation. If a duodenal tumor is detected or suspected, it is recommended to refer the patient to an advanced institution immediately.
