| 【 要旨 】 | 大腸癌は日本人において罹患数が2位，死亡数が3位となっている．欧米では大腸癌の罹患数と死亡数は減少してきているのに対して，日本の大腸癌死亡者数が高止まりしているのは，食生活の欧米化に伴う罹患率の上昇に加えて，検診受診率の低迷と6割前後の精検受診率が原因として挙げられる．精検の中心である大腸内視鏡をつらいと考え避ける人も多く，内視鏡を補完する新しい検査法である大腸CT検査（CT colonography）が注目されている．2012年に大腸CT検査が保険収載されたことから，大腸CT検査の積極的な活用と，さらには遅れている検査の標準化やガイドラインの整備，読影医の育成が急務である． | |

| [ Summary ] | Colorectal cancer (CRC) exhibits the highest direct incidence of cancer and is also the third greatest cause of cancer related deaths in Japan. Age-adjusted mortality rates for cases related to CRC have become high, because the incidence of CRC has increased. Rates for use of CRC screening or for those undergoing further tests are still low in Japan, compared to western countries. CT colonography is considered to be an alternative to colonoscopy for CRC screening due to high rates of tolerability. In 2012, the Japanese government (the Ministry of Health, Labor and Welfare) approved medical insurance coverage for CT colonography for CRC screening tests. This suggests an increasing use of CT colonography for CRC screening. In Japan, however, standardization of CT colonography procedures and education programs for readers are both lacking. Therefore, we should address these issues without delay. | |
