【 要旨 】 EUSガイド下腹腔神経叢ブロック（EUS-CPN）は癌性疼痛に対する治療オプションの一つであり，EUS関連手技のなかでは比較的容易かつ安全に行える手技である．その有効率は70〜80％とされており，有効性のさらなる向上を目指して，これまでさまざまな手技の改良の試みが報告されている．また，最近になって腹腔神経節そのものがEUSで描出可能であるという認識が拡がり，これを直接穿刺するEUSガイド下腹腔神経節ブロック（EUS-CGN）の有用性が報告されるようになった．腹腔神経節の描出に多少の慣れとコツを要するが，手技そのものはEUS-CPNと同様に容易に施行可能である．

[ Summary ] Endoscopic ultrasound-guided celiac plexus neurolysis (EUS-CPN) is an alternative method of relieving cancerrelated pain. EUS-CPN can reportedly relieve pain in 70 - 80 % of patients with internal abdominal malignancies. The procedure can be performed safely and easily. Several modified techniques have been introduced in order to improve the efficacy of this procedure. Moreover, use of the EUS technique, involving visualization of the celiac ganglia, has been reported to be feasible. The efficacy of endoscopic ultrasound-guided direct celiac ganglia neurolysis (EUS-CGN) has recently been reported. Although a certain amount of experience with the procedure is required to effectively visualize the celiac ganglia via EUS, the EUS-CGN procedure is as easy to perform as conventional EUS-CPN.
