【 要旨 】 第二世代超音波造影剤ソナゾイド®は低音圧にて共振するため,低音圧を使用するEUS に適している.造影ハーモニックEUS(CH-EUS)を用いると,膵充実腫瘍性病変を周辺膵実質の血流密度と比較することにより,Avascular(I型),Hypovascular(II型),Isovascular(III型)およびHypervascular(IV型)の4パターンに分類することができる.通常型膵癌の多くがII型を呈し,膵内分泌腫瘍の多くがIV型を呈する.膵囊胞性病変では病変の内部構造が明瞭化し,膵管内乳頭粘液性腫瘍においては粘液塊と腫瘍部の鑑別が可能となる.さらに,リンパ節やGISTの質的診断にも有用である

[ Summary ] The second generation ultrasound contrast agent, Sonazoid® can be oscillated by lower acoustic power levels. It is suitable for EUS because the smaller transducers employed with EUS produce limited acoustic energy. Based on the intensity of enhancement possible in pancreatic solid lesions, as shown by contrast-enhanced harmonic EUS (CH-EUS), they may be classified into four categories:avascular, hypovascular, isovascular and hypervascular lesions. CH-EUS depicts pancreatic ductal carcinomas as hypovascular nodules. In contrast, CH-EUS depicts most neuroendocrine tumors as a having hypervascular patterns. The internal structures of pancreatic cystic tumors, may be clarified with CH-EUS, making it possible to distinguish mural nodules from mucus clots in investigation of intraductal papillary mucinous neoplasms. CH-EUS is also used for evaluation of vascularity in organs other than the pancreas. Heterogeneous enhancement with CH-EUS displays features of malignant lymph nodes. When CH-EUS is used for assessment of the tumor vascularity of gastrointestinal stromal tumors (GISTs) CH-EUS can be used to identify irregular vessels in GISTs with high-grade malignancy.
