【 要旨 】 高度肥満症例の多い欧米を中心に，外科的肥満治療が急速に普及しつつあるが，ほぼ歩みをともにして外科治療と同様の治療効果が得られる内視鏡治療の開発が試みられてきた．長年にわたる創意工夫の結果，胃前後壁を縫い合わせ胃の容積を小さくする方法や，消化管壁表層を樹脂製のスリーブで被覆して栄養吸収をコントロールする方法など，さまざまなアプローチによる手技が開発され臨床的評価が始まっている．すでに短期の治療効果では，外科治療と同等の体重減少効果が報告されており，現在の開発の主眼は，いかに長期成績を向上させるかに移行しつつある．

[ Summary ] Bariatric sugery has been widely accepted in the West, where there is a high prevalence of severe obesity. Due to higher risks of adverse events in obese patients, endoscopic treatmetns have been explored as minimally invasive alternatives to surgery. An array of bariatric endoscopic treatments has been develeped and clinically evaluated. In this article, we review preliminary clinical outcomes from implementation of these technologies.
