The indications for artificial hydration and nutrition (AHN), including percutaneous endoscopic gastrostomy (PEG), for elderly patients and patients with dementia is a critical issue. However, reports in the press have given rise to misunderstandings. Compared to the West, the long-term prognosis for PEG is excellent in Japan. Therefore, it is necessary to keep that factor in mind with any discussion of these issues. On the other hand, dignity of life as an absolute value does not exist. It is, rather, a relative measure based on personal values. As such, the individualʼs views on life and death should be respected. Respecting the wishes of patients with advanced directives is important. However, it is desirable that patients create advanced directives voluntarily and are never coerced. |
