【 要旨 】 神経内分泌腫瘍(NET)の疫学,病態,治療などに関して,わが国では包括して討議される機会があまりなかった.2004年に膵・消化管NETに関する診断・治療ならびに研究に関わる最新情報の共有化,情報交換をはかり,本領域における臨床,基礎での研究の発展に貢献することを目的にNET Work Japanが設立され,日本で膵・消化管NETの実態調査,および全国疫学調査が施行された.2005年の膵神経内分泌腫瘍(膵NET)の受療者数は2,845人と推定され,機能性1,627人,非機能性1,218人であった.非機能性膵NETでは悪性率およびMEN1の合併率において欧米と大きな差を認めた.一方,消化管神経内分泌腫瘍(消化管NET)の受療者数は4,406人と推定され,部位別では中腸が少なく後腸が多く,欧米と大きな差を認めた.現在,2010年の膵・消化管NETを対象に第2回全国疫学調査が施行され解析中であり,5年間でどのような推移を示すか興味がもたれる.

Title Epidemiology of Neuroendocrine Tumors in Japan

[ Summary ] There have been few epidemiological studies on gastroenteropancreatic neuroendocrine tumors (GEP-NETs) conducted in Japan. The epidemiology of GEP-NETs [pancreatic endocrine tumors (PETs) and gastrointestinal neuroendocrine tumors (GI-NETs)] in Japan was investigated in 2005 using nationwide stratified random sampling. A total of 2,845 individuals received treatment for PETs. The prevalence rate was estimated to be 2.23/100,000 with an annual onset incidence of 1.01/100,000. Nonfunctioning tumors (NF)-PET constituted 47.4 % of those observed, followed by insulinoma (38.2 %) and gastrinoma (7.9 %). Multiple endocrine type 1 neoplasms (MEN-1) were detected in 10 % of PET scans but only in 6.1 % of NF-PET scans. On the other hand, an estimated 4,406 patients received treatment for GI-NETs. Prevalence rates were estimated to be 3.45/100,000 with an annual onset incidence of 2.10/100,000. The locations of GI-NETs varied, foregut, 30.4 % ; midgut, 9.6 % ; and hindgut, 60.0 %. The frequency of complications due to MEN-1 was 1 %. There are large differences in GEP-NET rates in Japan as opposed to Western countries, primarily due to differences in the presence of MEN-1 in NF-PET cases location, symptomatic status, and prevalence of malignancy in GI-NETs. At present, the epidemiologic survey of 2010 is on-going.
