【 要旨 】 経鼻内視鏡検査は近年スクリーニングを中心に広く行われるようになり，受検者の忍容性や安全性だけでなく，中・下咽頭病変に対する有用性も注目されている．経鼻内視鏡は近年急速に機能が改善されているが，経口内視鏡に比べ拡大機能がなく画質や送気・送水能が劣ることは否めないが，咽頭反射を起こしにくく，狭い咽頭腔での近接観察が得意であることなど有利な点も存在する．その利点・欠点について術者が十分に理解し，拡大経口内視鏡と経鼻内視鏡を検査目的や病変部位によってうまく使い分けることが重要である．また画像強調法，観察時の工夫としてのValsalva法や頸部回旋法などの活用が診断精度を向上させるために有用である．

[ Summary ] Transnasal small-caliber gastrointestinal endoscopy is now widely used to examine the upper gastrointestinal tract. Easy maneuverability in narrow spaces and comfortable examinations make transnasal small-caliber endoscopy preferable for observation of the oro-hypopharynx. Since patients do not gag or choke during transnasal small-caliber endoscopy, close-up view of the oro-hypopharyngeal mucosa can easily be obtained. Observation of the orohypopharynx by transnasal small-caliber endoscopy can be improved when combined with image-enhanced technology. However, the resolution obtained is lower than that with standard high-resolution endoscopes. For effective examination using transnasal small-caliber endoscopes, the Valsalva maneuver and the neck rotation method should be employed.
