【 要旨 】 経皮経肝胆道ドレナージと胆道ステント留置の適応と手技について概説した．経皮経肝胆道ドレナージは，偶発症や在院日数の面から内視鏡的胆道ドレナージ不能あるいは効果不良例が適応となることが多い．穿刺部位の選択は穿刺のしやすさではなく，その後の治療に適した部位を選択することが重要である．経皮的胆道ステントはプラスチックステントとself-expandable metallic stentがあり，原病の部位，良悪性，予後を慎重に検討して使い分ける必要がある．

[ Summary ] We described outline indications and procedures for percutaneous transhepatic biliary drainage (PTBD) and biliary stent placement. PTBD is often performed on patients who can not effectively be treated with endoscopic biliary drainage. Puncture site selection rather than ease, it is an important factor for suitable subsequent treatment. Percutaneous biliary stents include plastic stents and selfexpanding metal stents. We must select stents by the region of the stricture, site of the stricture, character of the involved region, and the prognosis.
