【 要旨 】 C型慢性肝炎に対するテラプレビルの治療により高い治癒率が得られるようになった．しかし，治療が失敗に終わった症例では耐性変異を有するウイルスが出現する可能性がある．耐性株の特徴は増殖能力がそれほど高くなく，インターフェロン感受性が高いことである．また，治療終了後経過とともに耐性株は消失していく傾向にある．さらに，出現した耐性が，現在開発中の第二世代のプロテアーゼ阻害剤には感受性である可能性も高い．C型肝炎ウイルス感染症例の多くが70歳近くになっており，好癌年齢が迫っていることも念頭に，治療を行っていく必要がある．

[ Summary ] The emergence of a telaprevir resistant strain of hepatitis C after failure of triple therapy is a concern for future therapy. However, it has been shown that the resistant strains are susceptible to interferon and gradually disappear. It is important to introduce therapy for patients with advanced liver diseases because they are at high risk of developing hepatoma.
