【 要旨 】 若年で発症する潰瘍性大腸炎（ulcerative colitis；以下UC）の経過において，妊娠・出産は大切な問題である．IBD寛解期の妊娠では，妊娠中再燃率は通常の再燃率と同等であるが，活動期の妊娠では，2／3で活動性が持続し，そのうち2／3が悪化するとされる．また，妊娠・出産回数に伴って，臨床活動性が低下するとする報告もある．十分な疾患管理のもとに寛解を維持して妊娠すれば，妊娠・出産がUCの臨床経過に悪影響を与えることはないと考えられる．

[ Summary ] At the time of conception, the risk of disease relapse from a quiescent state is the same as it is for non-pregnant ulcerative colitis patients. If conception occurs at a time when the disease is active, two thirds of the patients will experience persistent symptoms and of these, two thirds will deteriorate. It seems that pregnancy influences the overall course of UC positively, because clinical activity declines with pregnancy. Mothers with UC seem to have lower relapse rates in the years after pregnancy.
