| [ Summary ] |
We examined the long-term course of ulcerative colitis in 98 patients with pancolitis or left-sided colitis who were medically followed up for 10 years or longer. The annual incidence of active disease was 36 % in all patients, 6 % in those with an initial episode, 33 % in those with flare-ups and remission, and 75 % in those with chronic persistent disease. The incidence of active disease was thus high even in patients who were treated on an outpatient basis. At the time of disease exacerbation, proximal extension from leftsided colitis to pancolitis was found in 8 % of the patients. During follow-up, the hospitalization rate was 56％, the cumulative hospitalization rate was 51 % at 10 years and 58 % at 20 years, and the cumulative rehospitalization rate was 32 % at 10 years and 46 % at 20 years. Surgery was performed 10 or more years after disease onset in 3 % of the patients because of the development of rectal cancer.
