【 要旨 】 潰瘍性大腸炎の長期経過について,内科的に10年以上経過観察が行われた全大腸炎型,左側大腸炎型の98例を対象に検討を行った.症例全体の活動年率は36%,初回発作型6%,再燃寛解型33%,慢性持続型75%であった.外来治療例でも,活動年率が高い症例を認めた.症状増悪時に,左側大腸炎型から全大腸炎型へ,罹患範囲の口側進展を8%に認めた.経過中の入院率は56%,累積入院率は10年51%,20年58%で,累積再入院率は10年32%,20年46%であった.発症後10年以降の外科手術率は3%で,手術理由はすべて直腸癌の発生であった.その他の悪性腫瘍として異型上皮(dysplasia)3例,乳癌と悪性リンパ腫を各1例に認めた.

[ Summary ] We examined the long-term course of ulcerative colitis in 98 patients with pancolitis or left-sided colitis who were medically followed up for 10 years or longer. The annual incidence of active disease was 36 % in all patients, 6 % in those with an initial episode, 33 % in those with flare-ups and remission, and 75 % in those with chronic persistent disease. The incidence of active disease was thus high even in patients who were treated on an outpatient basis. At the time of disease exacerbation, proximal extension from leftsided colitis to pancolitis was found in 8 % of the patients. During follow-up, the hospitalization rate was 56%, the cumulative hospitalization rate was 51 % at 10 years and 58 % at 20 years, and the cumulative rehospitalization rate was 32 % at 10 years and 46 % at 20 years. Surgery was performed 10 or more years after disease onset in 3 % of the patients because of the development of rectal cancer. Other malignant tumors were dysplasia in 3 patients, breast cancer in 1, and malignant lymphoma in 1.
