Restorative proctocolectomy with ileal pouch-anal (canal) anastomosis (IPAA) is the treatment of choice for the majority of patients with ulcerative colitis (UC). The most common long-term complication from IPAA is pouchitis. Whereas acute pouchitis can be treated rapidly and successfully in the majority of patients, both refractory and chronic pouchitis remain therapeutic challenges for patients and physicians. Total colectomy with ileorectal anastomosis (IRA) is another surgical technique for UC, which has been performed less often since IPAA techniques were established. In patients with IRA, recurrence of inflammation or malignancy alternation in the mucosae of the remnant rectum are the most serious long-term complications. |
