【 要旨 】 NOTES（natural orifice translumenal endoscopic surgery）の臨床導入に当たっては，外科と内科が中心となって合同チームを編成し，基礎的検討を重ねておく必要がある．この過程では，双方の内視鏡治療に対する考え方やテクニック，使用機器の違いが鮮明になり，克服できないバリアの存在を強く認識することにもなるだろう．結果としてpureなNOTESではなくhybrid NOTESを選択することになったとしても，それは現行の内視鏡機器やテクニックの改善すべき点をはっきりと示しているとも考えられる．そして，NOTES臨床に挑戦する過程で浮き彫りとなった諸課題を，外科と内科が連携して解決していく姿勢こそが，今後のNOTES研究のあるべき方向性ではないかと考える．

[ Summary ] Before the clinical application of NOTES can be established, substantial basic laboratory work under multi-disciplinary team driven by GI surgeons and physicians is necessary. Differences between laparoscopic surgery and flexible GI endoscopy must be clearly recognized during this process. This challenge, will lead us to a deeper understanding of current technical barriers related to NOTES. Problems encountered during clinical attempts at developing NOTES will be overcome by close collaborative efforts by surgeons and physicians.
