【 要旨 】 膵管内乳頭粘液性腫瘍(IPMN)は腺腫より非浸潤癌，微小浸潤癌，さらにIPMN由来浸潤癌へと進展する疾患である．一方，IPMNの周辺の膵管に通常型膵管癌が異時性・同時性に併存することが報告されている．このようにIPMN関連膵癌にはIPMNより浸潤したIPMN由来浸潤癌(由来膵癌)とIPMNとは離れた膵管より発生した膵癌(併存膵癌)がある．また，IPMN関連膵癌はより早期に診断されることが多く，予後が良いことが報告されている．膵癌とIPMNが近接した場合，由来膵癌か，併存膵癌かの鑑別が困難となる．日本膵臓学会嚢胞性膵腫瘍委員会チーム3ではチーム2と協力し，由来膵癌と併存膵癌の定義(案)を考え，多施設より症例を集積し，その頻度と臨床病理学的特徴を検討した．その一部を概説した．

[ Summary ] Intraductal papillary mucinous neoplasms of the pancreas (IPMN) andrelated forms of pancreatic cancer consist of pancreatic cancer derivedfrom IPMN and pancreatic cancer concomitant with IPMN. The former frequently develops from IPMN of the main pancreatic duct and is sometimes mucinous carcinoma, while the latter usually occurs from branch type IPMN andis usually consideredto be tubular adenocarcinoma. IPMN related pancreatic cancer is usually detected in the form of smaller pancreatic cancer and the clinical course is better than that of ordinary pancreatic cancer.
