【 要旨 】 胃癌診断において拡大内視鏡が有用か否かをスクリーニングの視点と，ESD治療前の範囲診断の視点から当院の成績を検討した．まずスクリーニングの視点からは，通常内視鏡で癌か胃炎か判別不能であった105病変を検討したところ癌を感度86％，特異度85％で診断できた．また，範囲診断では157例のうち通常観察では53例が全周の明瞭な範囲診断は不可能であったが，そのうち44例はNBI拡大観察で全周の明瞭な範囲診断が可能になった．以上より，拡大内視鏡観察は胃癌診療に明らかに貢献していると考えられた．また酢酸を併用した「化学的」色素法は，拡大観察による範囲診断が困難な症例に有用なことも多い．その使い分けにも本論文は触れた．

[ Summary ] A study examining the usefulness of magnifying endoscopy for diagnosing gastric cancer from the viewpoint of screening and decision making before ESD is reported on. For screening endoscopic examinations, magnifying endoscopy could be used to diagnose gastric cancer with a sensitivity of 86％ and specificity of 85% for lesions which conventional endoscopy could not be employed to diagnose. In relation to decisions on the extent of gastric cancer before ESD, the extent of 53 lesions could not be determined with conventional endoscopy. However, 44 out of 53 could be determined with magnifying endoscopy. From these results, magnifying endoscopy was considered be useful for diagnosing gastric cancer.
