【 要旨 】 消化管間質腫瘍（gastrointestinal stromal tumor；GIST）の治療は，その概念の確立と分子標的治療薬の登場により治療体系が大きく変化した．GIST診療ガイドラインの発刊もあり，治療方針の組み立てが整理されたといえる．そのなかでも治療の第一選択は外科的切除であり，治癒の望めるGISTに対する手術は，消化器癌と同様に低侵襲性が求められるようになった．本稿では，GIST治療における低侵襲手術＝腹腔鏡下手術と，集学的治療における外科的切除の位置づけについて述べる．

[ Summary ] Treatment for gastrointestinal stromal tumors (GISTs) changed with the development of imatinib mesylate. Clinical practice guidelines for GIST in Japan illustrate that the first choice for treatment is tumor resection. Laparoscopic surgeryfor GIST and the status of surgical resection for advanced GIST are examined.
