【 要旨 】 診断のための内視鏡検査は安全に行われるようになった一方，ESDをはじめとする治療内視鏡はその拡大を広げ，食道，胃，十二指腸に至るまで行われるようになっている．これらによる偶発症である穿孔は時として致命的な合併症となるため，その診断から治療への判断はきわめて重要となる．本稿では食道，胃，十二指腸における内視鏡による穿孔に，どのタイミングでどう対処するべきかについて外科的治療という立場から述べた．とくに十二指腸の穿孔は致命的になるために，胃の穿孔とは重篤度が異なることを踏まえていただきたい．

[ Summary ] Today, accidental perforations during endoscopic examination are very rare. On the other hand, the incidence of iatrogenic perforations during therapeutic endoscopy such as EMR or ESD has increased because of the increased application of these techniques. Diagnosis and treatment of endoscopically induced perforations due to upper GI endoscopy is summarized.
