【 要旨 】 内視鏡診断，技術の進歩から内視鏡によって治療される消化管病変は増加している．それに伴い，より治療難易度の高いとされる病変も内視鏡的に治療されるようになった．内視鏡治療によるもっとも問題となる偶発症は穿孔であり，従来外科的治療を要した穿孔に対して，クリップを用いた閉鎖術の有効性の報告が多数なされている．内視鏡治療を行う医療者としては，同手技を習得しておくことは必須と考える．クリップによる穿孔閉鎖術が確実に行えれば，外科的治療を回避し，保存的に経過観察が可能となる．

[ Summary ] EMR has been widely accepted for treatment of early gastric and colorectal neoplasms. Perforations are the most severe complication associated with upper or lower endoscopic treatment. Recently, successful closure with endoscopic clips, without surgical treatment, has been reported. Immediate endoscopic clip closure and resulting conservative treatments are effective for treating perforations which occur during EMR or ESD.
