【 要旨 】 EBD・PTBD不能例に対する閉塞性黄疸の治療法として超音波内視鏡（EUS）下胆道ドレナージ術の報告が増えつつある．EUS下胆道ドレナージ術は安全，有用とする報告がほとんどであるが，実際のところ偶発症も少なくなく，サンプルサイズも小さいため，未だ確立された手技とはいえない．EBDやPTBD不能例には良い適応であるが，本手技を安全に行うためには新しいデバイスの開発が必要であり，EUSに熟達した医師，施設のもとで行われるべきである．

[ Summary ] Recently, EUS-guided biliary drainage for obstructive jaundice has been developed as an alternative to endoscopic transpapillary or percutaneous biliary drainage. Although several reports have showen it to be an effective and safe treatment for obstructive jaundice, they were done retrospectively with small sample sizes. Therefore, EUS-guided biliary drainage should be reserved for endoscopists at tertiary care centers with advanced training in EUS.
