【 要旨 】 2006年に国際膵臓学会ワーキンググループによる「IPMN／MCN国際診療ガイドライン」が発表された．ガイドラインでは主膵管型IPMNを全例手術適応とし，分枝型に関しては拡張分枝径＞30mm，壁在結節の存在，主膵管＞6mm，有症状例，細胞診陽性，のいずれかを満たすものを手術適応としている．一方，MCNの診断には卵巣様間質の存在を必須とし，malignantpotentialを有することから全例手術適応としている．現時点でのガイドラインとしては有用であるが，問題点も明らかとなりつつある．本稿ではIPMNを中心にガイドラインの概要を解説するとともに，改訂に向けての課題について述べる．

[ Summary ] The "International Consensus Guidelines for Management of Intraductal Papillary Mucinous Neoplasms and Mucinous Cystic Neoplasms of the Pancreas", was published in 2006. In this guideline, surgical resection is recommended for all main duct type IPMNs and branch duct type IPMNs which meet any one of the following criteria ； size of cystic branches >3cm, presence of mural nodules, dilated main duct (>6mm), or positive cytology. In regard to the diagnosis of MCN, histological affirmation of ovarian-type stroma is mandatory, and surgery is indicated for all cases. In this article, we outline the guideline and issues which should be discussed for future revisions.
