【 要旨 】 遺伝性非ポリポーシス大腸癌 (HNPCC) は,若年発症,大腸多発癌,多臓器重複癌を特徴とする常染色体優性遺伝性疾患であり,約90 %にmicrosatellite instability (MSI) を認める.DNAミスマッチ修復 (MMR) 遺伝子の異常がその原因である.診断は,revised Amsterdam criteriaやJapanese clinical criteriaに基づき,詳細な問診により行われる.また,revised Bethesda Guidelinesに基づき,MSIテストまたはMSH2 / MLH1の免疫染色の結果からMMR遺伝子のgermline mutationを検索する方法も有用である.

[ Summary ] Hereditary nonpolyposis colorectal cancer (HNPCC) is an autosomal dominant disorder characterized by early age of onset, multiple colorectal cancers, and the development of neoplastic lesions in a variety of tissues. Microsatellite instability (MSI) has been observed in about 90 percent of tumor tissue from patients with HNPCC. HNPCC is caused by germilne mutations in DNA mismatch repair genes. A diagnosis of HNPCC is performed based on the revised Amsterdam criteria or the Japanese clinical criteria and requires the taking of a detailed medical history. In addition, based on the revised Bethesda Guidelines, methods to search for the germline mutation of a DNA mismatch repair gene from the results of MSI testing or immunostaining of MSH2 / MLH1 are useful.
