EUS guided celiac plexus neurolysis has recently been reported on, focusing on it as a safer, more accurate method than the dorsal approach because of easier access to the celiac plexus, without any obstruction from intermediating organs. After depicting the celiac trunk from the gastric body, ethanol is injected through a needle punctured into the bilateral area adjacent to the celiac trunk. Confirming the presence of injected material by CT immediately after treatment improves accuracy. This method relieves pain in 78 - 88 % of patients with pancreatic cancer. No serious adverse events were reported, although complications such as temporal exacerbation of pain, hypotension and diarrhea sometimes occur. EUS guided celiac plexus neurolysis is expected to be a first line therapy for pain relief caused by abdominal malignancy. |
