【 要旨 】 内視鏡検診は対策型検診では推奨されていない．がん検診の有効性を評価するための指標は死亡率減少効果であり，単に多くのがんを見つけることががん検診の目的ではないためである．しかし，内視鏡検査は現在推奨されているX線検査と比較して小病変の拾い上げに優れていることや，早期の胃がんも診断できること，その場で生検も可能なことなどの利点があることから，任意型検診では内視鏡検査を採用する検診施設が増えている．内視鏡検診の課題として有効性の証明はもちろんであるが，受容性，安全性，マンパワーなど検査に伴う課題の解決が必要であり，学会主導で内視鏡検診の標準化に向けて準備が進められている．

[ Summary ] Screening endoscopy for gastric cancer has not been recognized a as method for organized screening. There are several problems with this technology such as safety, difficulty for patients and medical staffs, expense (more than treatments necessitated by lack of screening), labor intensive, low processing efficiently.
On the other hand, the demand for screening endoscopy has increased because of its high precision including biopsies, and for opportunistic screening. However, complete methodology has not been determined for things such as informed consent, preparation, sedation, observation, and photographing. Therefore, standardization of screening methods is very important. Safer techniques for endoscopists to create new technology with higher processing efficiently are required to provide adequate numbers of screenings.
