【 要旨 】 近年,内視鏡分野の進歩・普及はめざましいものがあり,胃癌治療においても早期癌の発見例の増加に伴い,内視鏡による低侵襲治療の役割はさらに高まっている.また,粘膜切除のみで外科的手術と同等の根治性が期待できる早期胃癌の特徴が明らかとなり,根治性の高い低侵襲治療として内視鏡的粘膜下層剥離術 (ESD) が考案され,普及しつつある.われわれの教室でも,ESDと命名される以前の切開・剥離法と呼ばれていた時期からこの治療法を導入し,早期胃癌の治療を行ってきた.教室におけるESD治療成績を示すとともに,早期胃癌に対する術前診断から術中および術後管理に至る治療戦略について述べる.

[ Summary ] Endoscopic mucosal resection (EMR)is a minimally invasive treatment for early gastric cancer. Recently, endoscopic techniques have been rapidly improved. Endoscopic submucosal dissection (ESD) is a new endoscopic surgery technique for en bloc resection of early neoplastic lesions in the gastrointestinal tract. Using this method, histological examinations can be performed more precisely, resulting in decreased local recurrence.
In our hospital, ESD has been performed for 250 cases of early gastric neoplasitc lesions. Histologically, curative resection was seen in 226 cases (91 %). Recurrence or metastasis was not seen in these cases. Local recurrence was seen in 2 cases. In both cases, the lateral margin was positive because of misdiagnosis of lateral extension. Major complications such as delayed bleeding and perforations was occurred in 2 cases (0.8 %) and 9 cases (3.6 %) respectively. All cases were endoscopically controlled without emergency surgery. It should be noted that not only are high quality incision techniques necessary but also diagnostic ability and post operative managiment for the performance of successful ESD.
