| 【 要旨 】 | 筆者らは，小病巣では内視鏡的粘膜下層剥離術 (ESD) での局所根治性を損なわずに安全・容易なstrip biopsyで代替できるものと考え，胃上皮性病巣におけるstrip biopsyとESDの使い分けを行っている．切除法の選択基準は，想定切除範囲2 cm以上はESD，1 cm未満はstrip biopsy，その中間の1 cm以上2 cm未満では術者の判断により適宜選択とした．中間集計での，サイズ制限を除くガイドライン適応条件内の早期胃癌および腺腫におけるstrip biopsy，ESD双方を合わせた1年局所再発率は2.7 %にとどまり，切除の約半数をstrip biopsyで代替しても局所再発の明らかな増加は生じないものと期待された． | |

| [ Summary ] | The appropriate uses of endoscopic resection methods such as strip biopsy and endoscopic submucosal dissection (ESD) for early gastric cancer may produce good outcome with low risk and low cost. It is most appropriate to perform ESD for lesions of over 2 cm and to perform strip biopsy for lesions less than 1 cm. For lesions over 1 cm and less than 2 cm, resection methods should be selected at the discretion of the attending endoscopist. Using such application standards, our one-year local recurrence rate after strip biopsy / ESD was 2.7 % of 75 lesions of gastric epithelial tumor. This result suggests that our application standard for strip biopsy / ESD is reasonable for early gastric cancer. |
