| 【 要旨 】 | センチネルリンパ節 (SN) 理論の概念は，決して新しいものではない．しかし，1992年にMortonらが歴史的報告をするまで，外科的臨床応用の可能性は十分には認められなかった．その後多数の研究が行われ，さまざまな悪性腫瘍でSN検出の重要性が証明された． UICCのTNM悪性腫瘍の分類 (第6版) では，「SNは原発腫瘍からのドレナージリンパ液が最初に流入するリンパ節である」と定義され，さらに「SNに腫瘍の転移を認めた場合，他のリンパ節にも腫瘍の転移が検出される可能性がある．逆に腫瘍の転移を認めない場合は，他のリンパ節には転移が検出される可能性は低い．ときには，SNは複数個存在する」と説明されており，これがSN理論の概念である． 消化管悪性腫瘍におけるSN生検の実行可能性と診断信頼性については，なお不明で議論があり，現在大規模な臨床試験が行われている． |

| [ Summary ] | The sentinel node (SN) concept is not new, but its potential surgical applications were not fully appreciated until the landmark report by Morton and colleagues in 1992. Numerous investigations have recently proved the importance of SN detection for various malignant tumours. In “TNM Classification of Malignant Tumours Sixth Edition”, SN is defined in the following way : “The sentinel lymph node is the first lymph node to receive lymphatic drainage from a primary tumour”. It goes on to describe the sentinel node concept as follows “If it contains metastatic tumour this indicates that other lymph nodes may contain tumour. If it does not contain metastatic tumour, other lymph nodes are not likely to contain tumour. Occasionally there is more than one sentinel lymph node”. The feasibility and diagnostic reliability of SN biopsy of gastrointestinal malignancies are still unclear and controversial. |
