| 【 要旨 】 | 非アルコール性脂肪肝炎 (nonalcoholic steatohepatitis ; NASH) とアルコール性脂肪肝炎 (alcoholic steatohepatitis ; ASH) とはその病理組織像が非常に似ていることから，共通の病態機序の存在が予想される．そして，実際に，ASHにおいて病態の主座をなすサイトカインの理解が，NASH病態機序の解明においてもきわめて重要である．また，一方で，NASH患者の多くは，その背景因子として，内臓脂肪型肥満に基づくメタボリックシンドロームの各因子を有し，メタボリックシンドロームの肝臓での表現形と考えられている．つまり，メタボリックシンドロームの病態を修飾する，脂肪組織由来のadipocytokineが，NASH病態にも大きな関与を有することが予想される． 本稿では，それらのサイトカインのNASH病態機序に果たす役割につき，概説することとする． |

| [ Summary ] | The similar histological fearutes and natural histories of alcoholic and nonalcoholic steatohepatitis (ASH and NASH) suggest that common pathogenic mechanisms may be involoved in these two conditions. Understanding the cytokine network involved in the pathogenesis of ASH should contribute to elucidating the pathogenesis of NASH. On the other hand, insulin resistance represents the most reproducible predisposing factor for NASH, and NASH has been appropriately considered to be one of the components of the metabolic syndrome, based on abdominal obesity. This suggests that adipocytokines secreted from adipose tissue may play a significant role in the development of NASH. In this review, we focus on the role of cytokines, especially related to ASH or metabolic syndrome, in the pathogenesis of NASH. |
