| 【 要旨 】 | 腸上皮化生を伴うBarrett食道は，腺癌の発生母地と考えられ，Barrett食道癌のサーベイランスに重要な所見であるが，通常内視鏡での判別は困難である． 拡大内視鏡観察時にpit patternを4群に分けると，Villous patternが腸上皮化生を伴ったBarrett食道の拾い上げ診断に有用であり，感度74.3 %，特異度78.0 %，正診率76.3 %であった。また，生検組織と対比では，Villous patternが腸型の粘液形質や細胞増殖能と関連していた． 拡大内視鏡観察時にVillous patternの有無に注意することにより，Barrett食道癌の早期発見が可能になると期待される． |

| [ Summary ] | A specialized columnar epithelium (SCE) associated with Barrett's esophagus, has been detected in random biopsies employing conventional endoscopy. However, magnifying endoscopy can be used to observe pit patterns associated with SCE in Barrett's esophagus. Targeted biopsies performed with magnifying endoscopy may contribute to the early detection of Barrett's cancer. | |
