【 要旨 】 Barrett食道は食道腺癌の前癌病変と考えられており，欧米では著しく増加しつつある．最近の研究成績は，Barrett食道は下部食道括約筋圧や食道運動機能の低下による胃酸やアルカリの逆流がその発生に関わっていることを示している．逆流症状の期間，逆流性食道炎の重症度，さらに性比，肥満，喫煙，Helicobacter pylori感染などの環境因子もBarrett食道の発生に関わっている可能性がある．本稿では，Barrett食道の発生に関する最近の報告を論じる．

[ Summary ] Barrett's esophagus is a premalignant condition related to esophageal adenocarcinoma and is becoming more prevalent is increasing in western countries. Several recent studies have indicated that Barrett's esophagus is related to increased acid levels, alkaline reflux, or both. This is due to decreased lower esophageal sphincter tone and reduced esophageal contractions. The duration of reflux symptoms, the severity of reflux esophagitis, and environmental factors such as sex ratio, obesity, smoking, or Helicobacter pylori infection play roles in the development of Barrett's esophagus. In this article we review recent studies on the pathogenesis of Barrett's esophagus.
