| 【 要旨 】 | 内視鏡治療技術の発達に伴い，術者に必要とされる技量は非常に大きなものに変化しつつある．とくに従来よりも大きな偶発症の発生が懸念される内視鏡的粘膜下層剥離術 (ESD) においては，その技量を向上させるため，簡単に実際臨床に酷似した形で習熟することができるデバイスが必要であった． われわれは，樹脂製の胃モデルとブタ切除胃切片を組み合わせることにより，前述諸条件を満たす内視鏡治療トレーニングモデルを開発した．これにあわせてESDの習得過程に，理解，習熟，上達の3段階を設定し，習熟過程にこれを利用した． 結果，実際臨床の場以外で簡単に手技の習熟をはかることが可能となり，安定した手技を提供できることにつながった． |

| [ Summary ] | Methods for endoscopic treatment of tumors of the digestive tract are steadily developing. However, these methods require a great deal of skill to be performed safely. The use of endoscopic submucosal dissection (ESD) has made it possible to remove large gastric tumors en bloc, and to achieve precise pathologic diagnoses. Sophisticated ESD techniques for handling the endoscope and forceps are required. The trainer for endoscopic treatment was developed to enable physicians to acquire these techniques out of clinical settings. This trainer has made it possible for physicians to acquire techniques efficiently, and has made practice easy. In our hospital, three stages of practice, (understanding, training and progress) in safe ESD operation were taught, using this trainer. This practice method was useful in acquiring greater ESD skills. Over 500 gastric tumors were operated on with ESD without any perforations occuring. The trainer for the implementation of endoscopic treatment will efficiently produce many skilled doctors. |
