| 【 要旨 】 | 内視鏡的粘膜下層剥離術 (endoscopic submucosal dissection ; ESD) の術者として正確な内視鏡診断学，確実な生検手技，そして穿孔，出血に対する内視鏡治療手技に精通する必要がある．また時として長時間に及ぶ症例があり，鎮静法や呼吸・循環動態などの術中管理も習得するべきである．その後，見学，介助を行い，胃前庭部のガイドライン病変から上級医のfull supportのもと術者として施行する．当センターの検討では，独力でESDを施行するのには30例の修練が必要だと考えられた． また内視鏡医，癌治療医として，術前のinformed consent (I.C) や術後の評価を正確に行う能力や，術者として一定の時間平常心を保てる，体力や精神力が必要である． |

| [ Summary ] | Endoscopic Submucosal Dissection (ESD) is widely used as a treatment for early gastric cancer. However, the techniques used are considered difficult for trainees. To master ESD, diagnostic techniques, therapeutic endoscopy, as well as sedative agents, respiratory and circulatory system are needed for trainees. Endoscopists who treat cancer should evaluate pathological results of ESD accurately and develop relationships with the patient and his or her family. We also reviewed ESD cases treated by trainees. There 11 ≤ cases ≤ 30 group, The complications observed were significantly higher perforation rate, and longer procedure duration. The median time was longer than in the other two groups (≤ 10, ≤ 50 experiences cases groups, respectively) In the guideline lesions. The one piece resection rate in the 11 ≤ cases ≤ 30 group was significantly lower than in the other two groups. To perform ESD by safely and accurately, trainees must perform at least thirty cases. |
