| 【 要旨 】 | Helicobacter pylori (H. pylori) 陰性時代における発生胃癌の特徴を予測するために，逐年内視鏡胃がん検診による胃癌発生頻度を算定した．血清H. pylori抗体 (HpAb) およびペプシノゲン判定 (PG) により以下の4群に層別化して検討した．A群 ; HpAb陰性かつPG陰性，B群 ; HpAb陽性かつPG陰性，C群 ; HpAb陽性かつPG陽性，D群 ; HpAb陰性かつPG陽性．各群の年率胃癌発生率は，A群0.04 %，B群0.06 %，C群0.13 %，D群0.60 %と算定された．H. pylori抗体価およびペプシノゲン値から考えると，H. pylori非感染例はA群と考えられ，今後BCD群の比率が減少し，A群の比率が増加することにより，胃癌発生頻度は低下していくと思われる． | |

| [ Summary ] | Helicobacter pylori (Hp) infection rates have been declining over the past 2 - 3 decades. We analyzed cohort studies on 6,983 subjects annually followed up on to assess possible changes in gastric cancer incidence in the future. We classified the cohort studies into 4 groups : according to Helicobacter pylori antibodies (HpAb) and pepsinogen status (PG) ; group A (HpAb− and PG−), group B (HpAb+ and PG−), group C (HpAb+ and PG+), and group D (HpAb− and PG+). During the follow up (4.7 years), 43 gastric cancers developed. The annual incidence of gastric cancer was 0.04 %, 0.06 %, 0.13 % and 0.60 % in groups A, B, C, and D, respectively. We established a super high risk group, males over 60 years in group D. They had the annual incidence of 1.8 %. Group A was assumed to have no Hp infection, whereas groups B, C, and D were infected with Hp. A decrease in Hp infection leads to an increase in the proportion in group A. Finally, it was projected that gastric cancer incidence will decline in the future. |
