| 【 要旨 】 | 発癌ハイリスクのBarrett粘膜として，特殊円柱上皮 (SCE) をもつもの，腸型ムチン形質を呈するもの，COX-2を発現するもの，dysplasiaを有するものなどが挙げられ，Barrett食道のサーベイランス法として色素内視鏡検査，拡大内視鏡検査，NBI内視鏡検査などの検討がなされている． 一方，Barrett食道の内科的治療薬剤として，プロトンポンプ阻害薬，アスピリン，NSAIDs，選択的COX-2阻害薬が検討され，さらに，Barrett食道を内視鏡的治療により脱落させ，その脱落した部位を本来の扁平上皮で再生上皮化させる試みもなされている． Barrett腺癌の増加が懸念されている現在，Barrett食道のサーベイランス法，Barrett腺癌の予防法を早急に確立していく必要があると思われる． |

| [ Summary ] | Some types of chromoendoscopy, magnification endoscopy or narrow-band imaging have been reported to have been used to effectively survey Barrett's esophagus cases. Targeted biopsies employing magnifying chromoendoscopy may contribute to early detection of Barrett's cancer. The value of PPI and aspirin / NSAIDs in the prevention of Barrett's cancer are still under investigation world wide. There are many reports on endoscopic ablation therapy for Barrett's esophagus. Further evaluation is warranted to define optimal methods and standardize procedures for diagnosis and management of Barrett's esophagus. | |
