| 【 要旨 】 | 明らかな隆起や陥凹を呈するBarrett食道表在癌の診断は容易だが，平坦型癌の診断は難しい．しかし，通常観察では境界不明瞭な平坦型病変であっても拡大内視鏡を用いると，その存在診断や進展範囲診断が可能となる． 拡大観察時に注目すべきは，血管パターンとpit patternである．血管の観察時にはNarrow Band Imaging (NBI) の併用が有用であり，口径不同，走行異常血管密度，ネットワークの形成に注目する．pit pattern診断には1.5％酢酸散布の併用が有用であり，pitの大きさ，形，密度，不整差から診断を行う． 本稿では実際の画像を基に，拡大観察を行うことで，平坦型Barrett食道表在癌が診断できることを示した． |

| [ Summary ] | The diagnosis of superficial Barrett's esophageal cancer has been difficult. Therefore, random biopsies have been recommended for the surveillance of Barrett's esophageal dysplasia in western countries. Magnifying endoscopy can be used to observe micro vascular patterns and pit patterns of associated lesions. And the characteristic vascular and pit patterns associated with superficial Barrett's esophageal cancer has been finding. In this paper, the authors exhibited many endoscopic views of superficial Barrett's esophageal cancer and showed the effectiveness of magnified endoscopy for the surveillance of superficial Barrett's esophageal cancer. |
