【 要旨 】 2000年,Iddanらによるカプセル内視鏡を用いた臨床研究が雑誌「Nature」に掲載され,以後カプセル内視鏡は,小腸疾患診断のモダリティとして全世界で汎用されるようになった.本検査法は全小腸の観察が高率で可能,検査中の苦痛がない特長がある.実際,他検査で診断がつかなかった原因不明の消化管出血が解決され,小腸クローン病,小腸腫瘍を検出でき治療が行われている.近年,食道用のカプセル内視鏡が臨床応用され,GERDやBarrett食道に対する有用性が認められつつある.保険適応後には国内でも盛んに行われることが予想され,新たな小腸疾患診断,病態の解明,またカプセル内視鏡技術の進歩が期待される.

[ Summary ] Wireless capsule endoscopy (CE) is a new, painless method of imaging the entire small bowel and has been used extensively around the world. CE has been reported many times to be superior to conventional endoscopy of the small bowel and was approved for clinical use in Europe and the United States in 2001. CE is said to be suitable for the first diagnosis of small bowel disease. With CE, many obscure digestive bleeding puzzles have been solved. Polyps and tumors that had not been detected before were demonstrated, and early lesions related to Crohn's disease was discovered.
In recent years CE for esophageal examination has been used clinically and is reported to be a convenient and sensitive method for visualization of esophageal mucosal pathology, providing an effective method to evaluate patients for esophageal disease.
After the soon expected approval of CE in Japan, it is thought there will be a great deal of use and development for diagnosis of small bowel diseases. It is expected that CE use will be accepted in Japan and come into wide use quickly.
