Colonoscopic reduction is a useful treatment of sigmoid volvulus and invagination. All patients with these conditions need emergent colonoscopy or emergent surgery to avoid necrosis or perforation of digestive organs. For some patients having this disease and who have sigmoid volvulus, or use tranquilizers and / or are elderly, we must first decide on the proper course of treatment. Secondly, the cause of invagination in adulthood is usually a tumor in the colon or small intestine. Therefore, the best treatment for this disease is resection of the tumor. Colonoscopic reduction, however, may be able to reduce the necessity of emergent surgery. We also need to take care concerning the indications for colonoscopic reduction for invagination in adulthood. Some investigators have pointed out the risks of perforation, cancer cell seeding into the peritoneum or metastasis in cases with advanced cancers. In summary, we never perform the excessive reduction of colonoscopy for these diseases. |
